Therapeutic indications:

Temozolomide is indicated in the treatment of:

    Patients with glioblastoma multiforme together with radiotherapy and subsequently as monotherapy Children 3 years of age and older, adolescents and adults with malignant gliomas, for example those with Glioblastoma Multiforme or Anaplastic Astrocytoma, who have Recurrence or Progression following standard therapy.

Mechanism of action:

Alkylating agent at physiological pH that converts to Monomethylthiazenoimidazole Carboxamide (MTIC) which is an active metabolite of dacarbazine; unlike dacarbazine, the formation of MTIC does not require metabolic activation. It is possible that the cytotoxicity of MTIC results from an alkylation at the O6 position of guanine, with additional alkylation occurring at the N7 position.

Administration and Dosage:

It should generally be taken once a day at night before going to sleep. Take temozolomide at the same time every day. Take the capsules with a glass of water and a clean stomach. In some classes of Brain Tumors, Temozolomide must be taken once a day for forty-two (42) to forty-nine (49) days. After a twenty-eight (28) day break, it may be taken once daily for five (5) days in a row, followed by a twenty-three (23) day break before repeating the next dosing cycle. The health professional is the one who determines how many times the treatment cycles should be repeated. How long the treatment lasts depends on the body's response and the type of cancer you have. Do not take more or less medicine than your doctor tells you to.

Contraindications:

Hypersensitivity to temozolamide, dacarbazine or their excipients.

Precautions: pancytopenia, myelosuppression, aplastic anemia, the risks are greater in geriatric patients, suffering from leukemia, myelodysplastic syndrome, hepatotoxicity, avoid pregnancy, pneumonia.

Adverse effects:

Nausea, constipation, vomiting, diarrhea, dizziness, mouth and throat sores, loss of appetite, headache, pale skin, weakness, loss of coordination or balance, fainting, hair loss, memory problems, insomnia, changes in view.

Serious side effects:

Red or black stools, urine that is either pink, red, or dark brown, unusual bleeding or bruising, coughing or vomiting blood, fever, constant cough and congestion, signs of infection, unusual tiredness or weakness, rash, inability to move on one side body aches, trouble breathing, yellow skin or eyes, seizures, right upper stomach pain, decreased urination.

Storage and disposal of the medicine:

Keep the medication in the original container, at room temperature, away from moisture and heat, tightly closed and out of reach of children. To dispose of the medication, a medication take-back program is recommended to ensure that children, pets or others cannot consume it.
